        ACP within integrated community wards , This role covers the four pillars of practice within integrated care , Role is highly autonomous and only staffed with ACPs out of hours , it involves a high level of autonomy and complex decision making . The role incorporates the four pillars of advanced practice , clinically involves medical clerk in and complete screen tools , patient assessment of undifferentiated diagnosis , advanced triage skills , managing the whole episode of care holistically and formulating a management plan , advanced clinical skills and diagnostics. The role involves independent prescribing and antimicrobial reviews and hospital avoidance . The role incorporates the non clinical pillars such as providing ACP teaching and Tacp supervision ,Quality and audit improvement work ,completing NICE reviews and Governance reviews . management of service evaluation. This role involves supporting SAFER/ and MDT in addition to completing ward rounds TTos and discharge summaries as well as referring patients for investigations or specialist services support and optimising management

        This was a hybrid role between ACP and Specialist that involved complex decision making with patients presenting to the Accident and emergency and Acute medical unit with wide undifferentiated diagnosis ,The role involved advanced clinical skills and including medical clerk in ordering investigations interpretation of results and creating medical management plans often for patients with wide differentials including frailty , multi organ failure , Respiratory . I was lead for respiratory in- reach and created a successful simulation teaching programme and patient pathways in collaboration with the accident and emergency team . This role involved audit such as NCEPOD, AQUA and NCAP for pneumonia and COPD bundles. I operated as the team leader and co- ordinated outpatient activity and off duty planning . I was a cascade trainer for the trust within this role and assessed staff competencies within AED and Respiratory . I presented regionally and contributed to research development to improve patient outcomes , this role involved Non – medical prescribing and optimising medical plans
